Maintenance requests
One channel, logged and traceable. Requests are sorted by urgency and risk rather than arrival order, and emergencies go straight past the queue. A request made by phone or in passing is not on the record, which is why the form matters more than it looks.
How to report it
A request sent through the proper channel is logged, dated and traceable. One sent as a text message to somebody personally can be forgotten, and if it is ever disputed there is nothing to point at.
Include what is wrong, where it is, when it started and a photograph if you can take one safely. That is usually enough for it to be triaged without a follow-up call.
Access matters too. Say whether a contractor can attend when you are out, because a repair waiting on an evening slot waits far longer. Notice rules still apply, as inspections and resources explain.
What happens next
Through the logged channel, or the after-hours line if it is unsafe or causing damage.
Sorted by urgency and risk of further damage rather than by who reported first.
Scheduled with you, or sooner without an appointment if you have authorised access.

What counts as urgent
No heat in winter, an active leak, no water, no power or a door that will not secure are emergencies. Report those to the after-hours line rather than waiting for the next working day.
A dripping tap, a loose handle or a slow drain are not, even though they are annoying. They are scheduled rather than rushed, and reporting them early is what stops them becoming the first list.
Do not arrange your own contractor. An unauthorised repair may not be reimbursed, and where a fault is a tenant responsibility it is recharged with the condition report supporting it. See resources and renewals.
